Document visits clearly and use observations to inform the care plan. Core principles Respect autonomy: let the older adult lead choices when able and honor preferences. Person‑first approach: focus on the person’s life story, strengths, and current abilities. Safety and dignity: prioritize physical safety without degrading independence. Active listening: give full attention, validate feelings, and avoid rushing responses. Observational vigilance: companionship visits are opportunities to notice clinical changes. Pre-visit checklist Review the client’s Service Plan and recent visit notes. Confirm any special instructions (allergies, dietary restrictions, mobility limitations). Bring essentials: ID badge, hand sanitizer, mask if required, small activity supplies (book, crossword, notebook, photo prompts). Ensure caregiver is rested, punctual, and mentally present. First 10 minutes: building rapport Greet by preferred name; introduce yourself with your role and a quick friendly line. Make eye contact at the client’s level; smile and use a warm, calm tone. Confirm comfort needs (temperature, seating, toileting) and ask permission before assisting. Offer simple choices to establish control (e.g., “Would you like tea or water?”).
